Moving Checklist for Primary Care Doctors in Georgia

12 Weeks Out
- Get at least three estimates from moving companies before signing a contract with a reputable one and picking a move date
- Move any patients scheduled during those moving dates to dates when the new office will be open, and reduce appointment availability the week before the move
- Notify all staff providers, CPA, patients, and referring physicians and hospitals both verbally, and through letters or other written documents
- Notify payors like Medicare and Medicaid
- Complete a change of address in printed materials such as Blue Book or any other place your office is listed
4-8 Weeks Out
- Notify any periodicals and subscriptions of your move, as well as your telephone and internet providers
- Notify your business suppliers (i.e. fax, water coolers) and medical suppliers (lab equipment) and set up a biohazard removal service (if needed)
- Notify your payroll service, bank, state license, DEA license, etc.
1-2 Weeks Out
- Start labeling the items that need to be moved and make sure to note the location that each item will be placed in your new office
- Pack the supplies that don’t need to be available for a few weeks
- Send reminder calls to patients about your new location
- Make sure you’ve acquired your Certification of Occupancy if moving to a new building
Prior to the Big Day
- Assign roles to your staff to be completed during the move
- Arrange to have phone, gas, and electricity hooked up and started in your new location a few days before moving
- Verify that all wall jacks, cabling, and outlets are active and that all lines are in place as needed
- Arrange for cleaning or anything else you need to have completed once you’re move out of your old location
